Due to new insurance laws (for patients on any insurance other than Medicare) please have your primary care doctor (PCP) send us a referral and obtain an authorization number from your insurance for us prior to making an appointment.

If you are a new patient calling this office, this office will not be able to give you an appointment date right away. Once your insurance information is taken down, it will be verified through your insurance and if active, the office will call you back to make your appointment. This process may take up to 2-3 days.
